If a club of landowner needs proof of insurance contact Starbuck, and if the club or landowner needs to be added to the insurance PM Starbuck with this information:
- legal name of club or landowner
- physical address
- mailing address if applicable
- contact name to receive certificate
- fax number or e-mail address to send certificate to
- dates of shoots

I have made a Blank Registration Sheet for the Shoot Bosses. After talking with coly, OG and D-fly, I thought it would be important to have this made up for everyone. This will hopefully help if there are any walk-ons and you need a sheet to write their names and all of their information down for Admin.
