I do not know if others have had this problem, but I will put this out there for general info.
This past weekend we had a student show up with a 15/22. She had been using it pretty regular, since she had also attended our recent boot camp. After about 8 sets of squares, she began to notice the malfunction. Upon careful observation, it was noticed that as she reset the sear the rifle would discharge. We called cease fire and immediately removed this rifle from the line, and replace with a loaner.
Once off line, it was feild stripped and upon inspection, found that not only was it firing at reset, but also when the safty was engaged. Further inspection found that the trigger pin and the hammer pin were both loose. They both had moved about 1/16th of an inch to the right. Just enough to be loose on the left side of the receiver. The pins were gently hammered bak in and function checks performed. After about 3 sets, the hammer pin slid out again.
The rifle was reassembled and tagged out, student was told that 1) the rifle needed to be seen by her gunsmith, Or 2) (my reccomendation) sent back to the manufacturer for repair/ replacement.
Hope noone else has had this occur but thought it was important enough to pass on.
My spidey sense is telling me S&W may have had a bad lot of lowers go out - not the first pin-related issue I have heard with their rimfire lowers this month. Not the second or third, either.
Conversation with a red hat leads me to believe that KNS antiwalk pins or similar will probably help a lot (which is a gigantic pain but at least they look really cool) - the safety thing is discomforting though. We should probably be keeping half an eye on them anyway given their early problems with OOBs and dumping extractors.
S&W will fix it pretty quick, they're not stupid. I have been wondering if they source those lowers externally or if they're made by Deep River in CT - if it's the latter, they've probably had it fixed for weeks or months already.
Thanks for the heads-up!
What is the chance that the trigger group had been removed and reinstalled and the springs were installed incorrectly or that they were installed wrong by the OEM?

After careful examination, the pin holes seemed a little enlarged making them loose. I am in contact(or will be) with this student to follow up on this.
Hi Colonial Shooter,
Yes, the AOC has received a number of reports concerning the M&P 15/22.
Please see the following advisory

I just read that it's no longer allowed, until further notice.

Is this true or click bait?

Could be. I had the same anomaly with an match/target AR. I swapped the OEM trigger for an aftermarket 2-stage trigger; it began discharging at reset. Cause: incorrect installation (springs not aligning with the notch on the pins.). After re-installation, works as advertised.
It's hazardous to shoot the messenger.
aj
apparently a OBD problem related to the extractor was well known a few years back?

Yes, it was.
Smith changed the bolt group a little to address it, but that reputation trailed the M&P 15-22 around for a while (deserved or otherwise).
