There is an updated Liability Release Form for 2012. It is two-sided and on the back of it are photography and model release forms. You can download the new form by going to the following link and download the "new liability waiver for 2012" PDF:

Please destroy any old liability forms in your shoot boxes and replace them with two-sided copies of the new form. When shooters check in to your events, please make sure they read and sign both sides of the form. Pictures from our shoots are one of our most effective promotional tools and we can't publish them if we don't have signed consent by the shooters (or their parents).
I appreciate all that you do to plan and execute Project Appleseed shoots. Let's work together on promotions to get more shooters on the firing line so we can help more Americans rediscover their heritage.

In our region the new liability waiver won't be included in shoot boxes until bob 210 runs out of the old ones per Fred's instructions.
Lawrence sent Reformed Redneck 1,000 copies of the new form, per Fred's approval, which we will hand out at the HBC this weekend. Hopefully these will tide us over until the old ones run out. If not let RR know and he'll have Lawrence send us more.
As a last resort print out the new one available on the link ShadowMan prosted.

How about putting the old ones through a printer to get the model release on the back? As far as I can tell, the only difference to the actual waiver is the range/location blank line added. (I'm in bob 210's area, and would also hate to just trash my stock of old ones.)

I suppose you may do that. The Legal team was concerned with the wording of the old waiver, thus the new one. It made sense to include the photography and model release forms on the back of the new one with the wording approved by the Legal team.
Bob 210 is still going to put the old wiavers in the shoot boxes for "CA, OR, NV, WA, UT and ID Supply Requests " per his discussion with Fred. If you wish to take the back of the new waiver and have it printed on the back of the old one I suppose that will be fine.
The Northwest region asked if we could use the new waiver. We were given permission so Lawrence is supplying them to us until the new waiver starts showing up in the shoot boxes.
Just for clarification:
The Northwest Region will be adopting the new liability waiver forms at all shoots effective April 21, 2012. New forms will be made available at the upcoming Bickleton, WA event. Also an electronic copy of the new form is attached to this post for your convenience.
Shoot bosses and instructors, please keep plenty of spare copies with you in case the old forms show up in your shoot boxes. Thank you for your cooperation in this matter.
